1. What coverage are you enrolled in?

A Marketplace plan (HealthCare.gov or my state's exchange)

Bronze, silver, gold, or platinum from the exchange.

An individual plan bought directly from a carrier

"Off-exchange" — same kind of plan, not through the Marketplace.

Medicare

Part A & B, Medicare Advantage, with or without Part D and Medigap.

My spouse's or parent's employer group plan

Coverage through someone else's job.

A short-term, limited-benefit, or health-sharing plan

Short-term medical, fixed-indemnity, or a ministry sharing program.

Medicaid, CHIP, TRICARE, or VA

Government programs other than Medicare.

COBRA from a previous employer

Continuation of a former group plan.
